Goals I’m working towards

Independence: I want to increase my impendence such as toileting, dressing and eating. Bimanual

skills: I am working on eating out of my lunch box, pulling my pants up and down, gluing and cutting.

Social skills: I want to learn to accept and understand my friends contributions and roles in different play episodes. I am also working on my reactions if I don’t win a game.

Communication: I want to be able to communicate effectively when I am anxious or when I am struggling with a sensory trigger.

Transitions: I want to be able to transition in the morning from my mum easier.

Pick up time: I am trying to not get so anxious when it gets closer to pick up.

My medical needs

Lexapro: I take anxiety medication, this is taken in the morning before school.

Sick Bay: I have severe anxiety about anything medical related, I get quite worried if other children are sick and i am very anxious at the thought of being unwell at school and sick bay. If i am unwell my mum will pick me up.

Adjustments/Modifications

Wheelchair: I use my wheelchair for long

AFO’s: These are my orthotics I need to wear them daily. If my feet get sore by the end of the day I can have a break.

What my diagnosis looks like for me:

Cerebral Palsy: I have Quadriplegic Cerebral Palsy this means that all four limbs are affected. My right side is more severly affected and I am not able to use my right hand too well. I can walk independently for short distances (on a good day), although I still do have falls. I use my wheelchair to conserve energy, when I am fatigued or for long distances. I need some assistance with day to day tasks but I am able to communciate when I need help.

Autsim: I am aware of my autism and understand some of the challenges I can face in different environments and situations. I sometimes find it difficult to communicate, be social, transition, regulate and cope with sensory triggers. I can get quite anxious about things which may seem insignificant to others. I am hightened.

The tricky stuff

These are things I struggle with and find challenging and here are some ideas of how you can help

Transitions: In the morning, at recess or lunch with different teachers.

  • provide me with a visual to help me understand when I am overwhelmed
  • use reassuring and repetitive language
  • offer a fidget toy or calm down toy

Regulation: Both related to sensory and emotional needs

  • therapy tool such as noise cancelling headphones or my chewy
  • take me somewhere quiet
  • refer me to my social story

Anxiety: I often get anxious about things that pthers may not even be aware of or notice

  • be patient and understand that I will communicate when I am ready and when I am able
  • offer constant reassurance and validate my feelings for me -make a plan with me on how we can overcome the worry

Motor skills: with my right hand, dressing, school work, sport or playing

  • give me ideas and strategies on how I can do it ‘my way’.
  • help me when I need it
  • praise me for my attempts

Ortotics: If my AFO’s are hurting me

  • Take them off while I am doing table work
  • Take them off and leave them off if it is at the end of the day
  • Not sure? Give my mum call
